What you need to know about Medicaid and the Healthy Michigan Plan

Apply online at michigan.gov/mibridges ORCall 1-855-789-5610 to apply.

Make sure to write down keep your MiBridges User ID, Password and the answers to your security questions.

After you apply. DHHS has 45 days to process your application. 

Open and read all mail from DHHS for requested verifications or decision letters.

Turn in all requested documents by the due date listed on the request (called a Verification Checklist or VCL).  Your application will be denied if you do not turn in the information requested by your DHHS caseworker (specialist) by the due date.  It is best to fax or upload your documents so you have proof you turned in the information.

You will receive a decision (determination) about your Medicaid health care coverage from DHHS. If you think the decision is wrong, contact the agency who helped you to file a hearing.

You will receive a yearly renewal (redetermination) that must be completed andreturned for your coverage to continue.  If you do not return the form, you will be   canceled and have to reapply.

Your mihealth Card  Keep this card. You will use this card as long as you are eligible for Medicaid.  You will get a green mihealth card in the mail from DHHS when you first become eligible for Medicaid. Take this card with you when you go for medical care.  If you need a new card, call 1-800-642-3195. 

Call MI Enrolls 888-367-6557 After you are enrolled in Medicaid, you will get a letter from MI Enrolls to select your Primary Care Physician (PCP) and pick a Medicaid Health Plan. Medicaid Health Plans are listed on the other side of this sheet.


Medicaid Health Plans

These plans are available in the Washtenaw, Livingston, Jackson, Hillsdale, Lenawee and Monroe counties.

You will receive a letter stating your Medicaid is active. 

Call your doctors and ask them which Medicaid health plans they take.

You have 90 days to change your health plan. 

You can change your Primary Care Physician at any time.
